The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
Schedule: Monday-Friday, 8am-5pm

Minimum Education Requirements
Bachelor of Science in Nursing (BSN), required
Minimum Experience Requirements
3 years of experience required, with one year in ortho/spine
Strong psychosocial assessment and clinical skills
Service line experience as a nurse navigator, preferred
License/Certifications
Current state RN license
Current Basic Life Support certification from American Heart Association
Board Certified RN with the American Nurse Credentialing Center within specialty, preferred
Physical Requirements
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
