Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health
issues for patients; linkage to resources in the community.

What You Bring/Job Requirements

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

 

6 months home care/hospice experience preferred.
Two years post Master’s experience in health care or related field preferred.

Master of Social Work (MSW)

LCSW Preferred

Current Driver’s license

BLS

Medium Work - (Exert up to 50lbs force occasionally, and/or up to
20lbs frequently, and/or up to 10lbs constantly)

 

Varied schedule. Needing help with weekend MSW support, potentially overnight on call help, as well as working with our admission team with consent signing and discussion of hospice philosophy.
