    The transplant social worker provides comprehensive social work services to patients and families through all phases of transplant—from date of initial referral through transplant hospitalization and for the duration of the post-transplant period. As a member of a transplant team, the social worker provides patient education and emotional support, is responsible for referrals to community agencies, coordinates patient care with other team members, and assists with concrete services. Assesses the patient’s physical, psychosocial, cultural and spiritual needs. Assists in the coordination of appropriate services, resources and referrals. Advocates and provides support to patients and families who are having difficulty coping effectively with changing medical conditions. These individuals are necessary and critical in the assessment of transplant patient candidacy. As such, their evaluations and recommendations can be determinant as to whether a patient is listed for a chance at life-saving therapy or denied the opportunity.

    What You Bring/Job Requirements

    In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

    • High school diploma or GED.
    • Graduate of Accredited Master’s in Social Work Program.
    • Certified Clinical Transplant Social Worker (CCTSW), preferred.
    • Previous transplant experience is, preferred.
    • 2 years of social work experience in a hospital or health care setting.
    • 5 years Social Work experience; preferably in a clinical setting.
    • Previous experience in acute care, homecare and long-term facility care experience, preferred.
    • Third party payment systems, Medicare/Medicaid programs, preferred.
    • Knowledge of discharge planning and care management.
    • Knowledge of state and federal governmental agencies’ roles as they relate to healthcare.
    • Knowledge of Mandatory reporting requirements.
    • Maintain professional licensure available to social work healthcare practitioners in the practice locale.
    • Special requirement: LCSW required

    Physical Requirements - Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
